Ten years ago, when I first started engaging in the public welfare activities, I met Ren Nengping and Ren Nengli. They were brothers bonded by blood, yet they were also bound together by a similar misfortune, both diagnosed with advanced pneumoconiosis after working as miners for years.

Between the two, Ren Nengping, the younger brother, first received my medical assistance to undergo a lung transplant. During his surgery, I was in the operating room, witnessing the entire procedure, and I even touched with my own hand the diseased lung removed from his body. It was no longer a lung anymore but broken pieces of hard black coal-like fragments. Then, a new healthy lung was placed in his chest after traveling a long and complicated journey. Harvested from a brain-dead donor in Kunming city, Yunnan province, this valuable organ was soundly preserved and immediately transported by air to the hospital over a thousand miles away in Wuxi city, Jiangsu province. Every step of the transportation, including the landing of the plane that carried the organ, must be achieved according to strict timelines, for any delay would be fatal for saving his life.

Unfortunately, although the transplant was completed through all the challenges, Ren Nengping passed away the next day. The fact was that Nengping’s health condition had been no longer eligible for such a major operation. However, out of his strong desire of survival and my best wishes to extend his life, we decided to give it a try nonetheless and strive for a miracle. At that time, I was inexperienced in pneumoconiosis assistance, which led me to place too much confidence in the effect of human’s willpower. As a result, I was already mentally preparing myself before Nengping’s surgery for the high cost of post-operative treatment to minimize the risk of organ rejection.

Ren Nengping’s death proved that one’s determination plays a much less decisive role than his or her real physical condition in the treatment of pneumoconiosis. Since then, I have been wondering whether it is the right choice for pneumoconiosis patients to undergo a major operation like a lung transplant when they are already in the advanced stage of the disease. Moreover, in consideration of its various limitations, including risks, high expenses, operative difficulties, and a short supply of organ donors, lung transplantation may not be a suitable solution to meet the medical needs of the still-growing number of people suffering from pneumoconiosis. Faced with these problems, the YuanLi Foundation has made great efforts to raise awareness of hazardous dust exposure on work sites besides funding for curative care. We believe that “an ounce of prevention is worth a pound of cure”. Only by preventing this disease from occurring in the first place can we truly eliminate it. However, major firms that involve dust-exposed work refuse our attempts to promote awareness of pneumoconiosis among their workers, using the excuse that they have already implemented comprehensive employee protection measures.

Meanwhile, many small workshops engaged in such work remain hidden from public view, making it difficult for us to even locate them. All these companies regard the Foundation as a “disrupter” to their business. Alas! How sincerely we hope for an opportunity to teach people there about pneumoconiosis and raise awareness of it! Then we can shift from being passive to being active, helping more people at risk of occupational dust exposure, in addition to those poor patients who are already in the late stages of the disease.

Ren Nengli, the elder one of the two, died not long after his brother’s death. I happened to visit him on the last day of his life. He had always held out hope that he might regain his health through a lung transplant and asked me for help. However, considering the failure of his brother’s surgery, I didn’t dare to take any risks. I asked him if he blamed me for rejecting his request. To my surprise, his lips curved into a weak but soft smile. “No,” he said with his last bit of strength, and this was his final word to me. Nengli left the world early the next morning. In the small hours, I was awaken from my sleep by his wife’s phone call which informed me that he had passed away in the hospital and would be brought home overnight for burial. As I was staying in a hotel nearby his village when the call came, I rushed to Ren Nengli’s home and accompanied his family during the funeral.

Over the past decade, many of the migrant workers afflicted with pneumoconiosis and assisted by the YuanLi Foundation have passed away. Through the Foundation’s aid projects, I came to know these people and established close friendships with them. I witnessed the last sparks of their lives and mourned for each and every one of their departures. Even today, I can still remember their faces vividly. These good people died young with hope for a future unfulfilled. They had never did anything socially harmful throughout their lives, but they and their colleagues — the hundreds of millions of Chinese migrant workers — have greatly contributed to our nation’s rapid economic growth since the reform and opening-up. We should appreciate their contribution. These people lived ordinary lives, and then died quietly like leaves falling from a tree, yet each fallen leaf is unique and has its own name that should be memorized.

On Tomb-sweeping Day, I’d like to express my heartfelt condolences to them by borrowing the words that are frequently used by these pneumoconiosis patients in their chat groups when someone passes away: “Farewell, my friends. May you be free from pneumoconiosis there in heaven. May your souls rest in peace.”
